A 80 year old female presented with a 2-week history of cough and shortness of breath. She also reported significant weight loss and loss of appetite over the past 2 months. The patient was not maintaining O2 saturation on room air and required about 4l/min O2. General physical examination revealed hard, multiple matted cervical and supraclavicular lymph nodes. Other vital signs were within normal limits. On auscultation, there was b/l extensive rhonchi+. Cardiac, abdominal, and neurological examination was normal. A chest radiograph was ordered [Figure 1]a. And CT Thorax was done [Figure 1]b.
