Corona, vaccination, second wave, what next?

My Dear Colleagues,

We were having a sigh of relief with active Corona cases coming down in the month of January and February 2021 and routine activities beginning to function. The Nation as such had started to recover from the onslaught on all fronts.

The vaccination drive began in January 21 starting with the front line warriors, but its journey was far from being a smooth affair. The first thing was that it was made optional. There was a lot of hue and cry from the medical fraternity itself. It was stated that the clinical trial of one of the vaccines was incomplete and until the final result was out, it should not be used. It was indeed true at that time, but it sowed the seed of indecisiveness to take the other vaccine whose trial was complete. This resulted in <50% coverage in the stipulated time. The next phase was for all those who were 60 years and above and those in the age group of 45–60 with comorbidities. There were a few incidences of adverse events, a few reported deaths which were not confirmed as due to vaccination and a few cases of coagulation abnormalities. Which vaccine is 100% safe? Is there any vaccine which has not produced some side effects in the past or present? The media both visual and print, highlighted the side effects issue so much, it sowed the seeds of doubt in the minds of the public as to take the vaccine or not. The side effects were debated day in and out but not the advantages of taking the vaccine there by putting a break to the pandemic. No vaccine can prevent the disease from occurring in Toto and there is no rule that those who got vaccinated cannot get corona. Even if somebody gets corona after proper antibodies have developed after vaccination, it would be mild and won't take the life. The mutated virus has been infecting patients rapidly and doubts have arisen whether the current vaccine would be efficacious against it. Some components of virus have shown mutation and the present vaccines efficacy against them would be there but may not be complete. When the pandemic is dragging for more than a year, mutations are bound to happen and new vaccine cannot be manufactured after every mutation.

Once the number of cases stared to reduce, Governments relaxed the rules and the public completely forgot the Sanitation, Mask, Social Distancing (SMS) they had to follow and now a second wave of Corona is sweeping the country. People are still adamant to follow the rules. It has come to that extent that fines are being levied for not wearing masks, not keeping the safe distance and for excessive congregation of people. People should change their attitude and follow the safety precautions. No epidemic or pandemic will end without the change of attitude of the people.

Could not have the Government made vaccine compulsory in the interest of the Nation? Can the media change their attitude of negativity and promote the positive effects of vaccine? And repeatedly hammer about the COVID saftey precautions instead of the deaths that have occurred? These questions are unanswered at present but need to be answered. How would you expect this pandemic to end? Either there should be herd immunity which probably would lead to more and more deaths, or vaccine induced immunity to break the chain in the spread. Meanwhile, the public have to scrupulously follow the SMS and get themselves vaccinated early to save themselves and the others. The change of attitude of the public is the biggest thing that should happen early, otherwise no salvage is seen in the near future.

Coming to the present issue, there is a review article on “Adult Vaccination” with a guest editorial by Dr. Praveen Kulkarni on the same subject. Probably, this is the subject of the day. The other review article on “Headache” does not give any headache to the readers but gives very useful information to the clinicians. We have a host of original articles, and the regular features of electrocardiogram diagnosis, Pictorial CME, etc. Do send your observations after going through the articles and your feedback would definitely help us to improve the standard of articles.
